Targets and End Points in Cardiac Autonomic Denervation Procedures

چکیده
Autonomic system modification is an established therapeutic approach that has been increasingly used in several arrhythmic disturbances. Ganglionated plexus (GP) ablation for the treatment of vagal-related atrial fibrillationand symptomatic bradycardia, such as cardioinhibitory vasovagal syncope, has been widely studied, and although convincing results had been reported by some authors, no consensus still exists on the most adequate technique to identify the critical targets or end points indicating clinical vagal attenuation.

BACKGROUND Autonomic denervation is an alternative approach for patients with symptomatic bradycardia. No consensus exists on the critical targets and end points of the procedure. The aim of this study was to identify immediate end points and critical atrial regions responsible for vagal denervation. METHODS AND RESULTS We enrolled 14 patients (50% men; age: 34.0±13.8 years) with cardioinhibi...

The cardiac effects of ethanol were studied in six conscious dogs chronically instrumented for measurement of left ventricular pressure, internal transverse diameter, and outflow. Successive intravenous infusions of ethanol produced blood levels of 120 16 (SEM) mg% (infusion #1) and 311 19 mg% (infusion #2). Stroke volume decreased from 20.1 1.6 ml preinfusion to 17.2 1.9 ml after infusion #1 (...

one of the important complications of diabetes mellitus that physicians pay less attention to is diabetic autonomic neuropathy(DNA) ,especially cardiac autonomic neuropathy (CAN).this has a poor prognosis and is accompanied by sudden death risk, recently several investigations that were carried out for CAN detection ,suggested that QT-interval prolongation in these patients is an important diagn...

Evidence for vagal denervation of the heart as a feature of diabetic autonomic neuropathy has been obtained by monitoring beat-to-beat variation in heart rate. Nine diabetics with autonomic neuropathy were assessed; each showed a marked reduction or absence of beat-to-beat variation in comparison with controls. Beat-to-beat variation in normal subjects is abolished by parasympathetic blockade b...

Signs or symptoms of impaired autonomic regulation of circulation are typically present in patients affected by Parkinson's disease (PD), in agreement with the cardiac sympathetic denervation discovered by Goldstein more than 15 yrs. ago. In particular, the majority of PD patients have a diffuse left ventricular myocardial sympathetic denervation, being a normal neurological condition present o...
